📄 How to Read Your Patient Statement

We created this guide to help you understand your Patient Statement step-by-step.

Your statement shows all the recent activity on your account, including visits, charges, payments, and your current balance. Here’s how to read it:


🔹 1. Patient Information (Top of the Page)

At the very top, you'll see:

  • Your name and address

  • The dates this statement covers

  • Your account number

👉 Look here first to make sure your personal info is correct. If something’s wrong, call us at (612) 354-2592 or email us at billing@reedandember.com so we can update it.


🔹 2. Statement Summary (Upper Right)

This section gives a quick look at your account, including:

  • The total amount you owe

  • Any charges from recent visits

  • Payments made by you or your insurance

  • Adjustments (discounts, corrections, etc.)

👉 Look here to see how much you owe right now and if you’re on a Payment Plan.


🔹 3. Account Summary (Middle of Page)

This part lists all the details from your account, like:

  • Dates of services

  • Charges

  • Payments made

  • Adjustments

  • Your current balance

👉 Look here if you want to see each transaction listed one by one.


🔹 4. Forwarded Balance Detailed Summary (Bottom of Page)

This section shows any past unpaid invoices that were brought forward to this statement.

👉 Look here to see a list of older balances that are still unpaid. These can sometimes be added up to your total amount due.


🔹 5. Final Amount Due (Bottom)

Here’s where you’ll find the total amount you owe as of the date on this statement.

👉 This is the final number to focus on if you want to know exactly what to pay.


✏️ Special Note Section (Very Bottom)

If there's something important we need you to know—like missing insurance info or account changes—you’ll see it in a “Note” section at the very bottom of the statement.

👉 Only shows up if there’s something special you need to read or do.


💳 How to Make a Payment

You can pay your balance in one of three easy ways:

📨 Option 1 – Email Authorization

Send an email to billing@reedandember.com with this sentence:

“I authorize Reed & Ember PLLC to pay [ENTER AMOUNT HERE] using the payment information stored on file.”

💡 Example: “I authorize Reed & Ember PLLC to pay $100.00 using the payment information stored on file.”


🌐 Option 2 – Pay Online

Go to this secure link and click the “Pay Now” button:

🔁 Option 3 – Auto Pay

Send an email to billing@reedandember.com with this sentence:

“I authorize Reed & Ember PLLC to automate recurring charges using the payment information stored on file.”

Auto Pay means your account will be charged automatically one day after your session or one business day after you authorize it. No need to call or email each time.

🛑 You can stop Auto Pay any time by emailing us and asking to cancel it.
